Sarah Hyland poses topless as she opens up about body transformation

She's been candid with her journey of overcoming several health issues. 

And it looks like Sarah Hyland is ready open up even more, taking to social media on Wednesday to pose topless for a new Skin Transformed campaign with Olay. 

The 28-year-old Modern Family star said her collaboration will see her featured in a waterproof bath book as she continues to strive for progress and not perfection with her body.

Speaking with Self, the beauty showcased her many stomach scars before opening up about her failed kidney transplant.  

However towards the end of 2016, the kidney began to fail, for reasons that are still unknown.

'We did all of these tests and all of these treatments to try and save the kidney, but they basically said that the transplanted kidney was like a house that caught on fire. You can't unburn a house,' she said.

The beauty then underwent dialysis three times a week - each for four hour sessions between February 2017 and October 2017.

It was in July 2017 that she found out that her younger brother Ian was a match. 

'When a family member gives you a second chance at life, and it fails, it almost feels like it's your fault. And it's not, but it does,' she tearfully confessed.

'When a second family member, someone, you're supposed to look after, like a little brother, wants to give you a third chance at life, it's scary, because you don't want to fail them.'

Thankfully, Sarah confirmed during the interview that there's been no rejection to the second kidney transplant and that she is 'healed and the numbers are good'.

On Valentine's Day of 2017, Hyland went on dialysis, causing her weight to plummet, before she had the transplanted kidney removed in May 2017.

Sarah added that the physical ailments had a huge effect on her mental health too, and she even contemplated suicide.

'For a long time, I was contemplating suicide, because I didn't want to fail my little brother like I failed my dad.'

Sarah didn't specify what the pains were from in the post.
